All purchases must be made on one receipt, and the UPC codes must be written on the form if the product name is not on the receipt. You have to purchase the items between 1/14/08 - 3/31/08 and submit the one original cash register receipt. The form must be postmarked by $4/15/08 and received by 4/22/08, and it is a limit of one per household.
